Follow-Up – Telehealth
I have been seeing you for gender affirming services and I need to follow up with my provider. I would like to have a Telehealth appointment.
Injection/Vaccination
I need my next Depo injection. This shot is on time and I am not having problems. If I am having problems or if I am due for my yearly visit, I will call the office to schedule.
Medication Check
I would like to speak to my provider about medications I am taking or to start/change my birth control method (including Depo). I will call the office to schedule if I would like an IUD or a Nexplanon insertion or removal.

Problem
I need to see my provider regarding some other issue or concern. For example: I have symptoms and I would like STD testing, I need to check my blood pressure, I have a bump, I have abnormal bleeding, I have an infection, I would like a breast exam, I have a UTI/bladder infection, etc.

Testing

Wellness Visit
It is time for my yearly wellness visit (It has been at least 366 days since my last wellness visit). I will call to schedule more time if I have a problem I would like to discuss as well.
